Ohalah Membership Applications

Please copy the application that is pertinent to you, paste it into your word-processor, complete it, print and mail to:
OHALAH c/o Rabbi Yocheved Mintz, 2045 Grouse Street, Las Vegas, Nevada 89134. Application fees are set forth on the top of page 2 of each application.  There is no fee for changing your status from student to full member. The Ohalah Membership Committee meets quarterly to approve new applicants, at the beginning of January (at Ohalah), April, July, and October (before Ohalah scholarship deadline). Please try to submit your complete application package by the beginning of the preceding month (March, June, September, December). An application can be expedited in special circumstances.
